Overnight (00:47 UTC 6/8 to 00:47 UTC 6/9), BP values were largely reassuring except for two critical spikes. After a normal reading at 20:44 (120/80, HR 70), there was a severe hypertensive crisis at 20:44 with BP 199/198 (HR 197), crossing ACOG Stage 2 thresholds (=160 and/or =110). A subsequent reading at 20:47 improved to 135/80 (HR 100), then again at 20:48 there was another severe Stage 2 event with BP 220/150 (HR 100). By 21:35 BP returned to 118/76 (HR 72).

AI severity mirrored these events (Severe at 20:44 and 20:48; Moderate at 20:47; Low at 20:44 baseline and 21:35). No symptoms were detected per AI, and no EPDS trends were available. Two “Severe Alert” events were generated at 20:45 and 20:48 and both were acknowledged with SLA met. Urgent SMS guidance was sent at 20:45 and 20:48 to seek emergency evaluation (ER/911), with a follow-up message at 20:47 indicating stability but continued vigilance.

ACOG Stage 2 threshold crossing occurred at 20:44 and 20:48; subsequent readings demonstrated rapid normalization by 21:35.

Overnight BP monitoring (00:42 UTC–00:42 UTC) showed intermittent extreme elevations with subsequent normalization. At 20:44 UTC, a severe reading was recorded with BP 199/198 (pulse 197), crossing ACOG Stage 2 (=160/=110). Care alert was acknowledged (SLA met) at 20:45 UTC, and an urgent SMS message advised immediate ER evaluation/911. A follow-up at 20:47 UTC showed BP 135/80 (pulse 100), representing improvement but still within the 1st-trimester? (stage documented as second trimester) without symptoms. Another severe spike occurred at 20:48 UTC with BP 220/150 (pulse 100), again crossing ACOG Stage 2; a second severe alert was acknowledged (SLA met) and another urgent SMS was sent at 20:48 UTC. By 21:35 UTC, BP decreased to 118/76 (pulse 72) with low-severity assessment and no detected symptoms. No EPDS data available and no escalation/resolution beyond alert acknowledgment and BP normalization by 21:35 UTC.

Overnight BP surveillance (00:37–next 00:37) showed critical transient hypertensive readings despite later normalization. Notably, at 20:44 BP recorded 199/198 with pulse 197, triggering a severe alert (acknowledged; SLA met) and SMS urging emergency evaluation. Subsequent readings included 20:47 BP 135/80 (stage 1 threshold not crossed) and 20:48 BP 220/150 with pulse 100, again exceeding ACOG Stage 2 (=160 systolic and/or =110 diastolic), prompting a second severe alert (acknowledged; SLA met) and a second urgent SMS. By 21:35, BP returned to 118/76 (normalized; ACOG thresholds not crossed). Overall trend: severe spikes at 20:44 and 20:48 followed by stabilization and resolution of extreme values within ~47 minutes. No symptoms were detected in the AI reports (symptom score 0); EPDS not reported. No escalation orders or medication changes are documented in the provided dataset; severe alerts were acknowledged and correspondence was sent, with resolution only in terms of BP normalization, not confirmed clinical management.

Overnight (00:32 UTC 6/8–00:32 UTC 6/9), there were two critical BP excursions with severe readings despite no reported symptoms (per AI symptom screen).

Most concerning: 20:44 UTC BP 199/198 (pulse 197) followed immediately by continued severe hypertension at 20:48 UTC BP 220/150 (pulse 100), both clearly crossing ACOG Stage 2 thresholds (=160 systolic and/or =110 diastolic). These triggered Severe Alerts at 20:45 and 20:48, both acknowledged with SLA met.

Other measurements were less concerning and transient: 20:47 UTC 135/80 (pulse 100) and 21:35 UTC 118/76 (pulse 72), suggesting the extreme values were episodic rather than persistent. No EPDS data available; no escalation resolution documentation beyond alert acknowledgment.

Patient messaging: SMS sent at 20:45 and 20:48 with urgent ER/911 instructions; an additional SMS at 20:47 indicated status was steady and requested continued vigilance.

No further severe BPs recorded through 21:35 UTC.

Overnight (00:01 6/8–00:01 6/9 UTC), BP was largely normal with two critical hypertensive spikes. Initial reading at 20:44 showed 120/80 (p70), then immediately at 20:44 a severe outlier occurred: 199/198 (p197), exceeding ACOG Stage 2 threshold (=160 and/or =110). A subsequent reading at 20:47 was 135/80 (p100) with improvement toward normal/stable. At 20:48, BP rose again to 220/150 (p100), again clearly Stage 2 (=160/=110). By 21:35, BP returned to baseline range at 118/76 (p72).

Two “Severe Alert” events were generated (20:45 and 20:48) and both were acknowledged with SLA met; no symptoms were detected per AI assessments (0 symptoms across all readings). Care escalation messages were sent at 20:45 and 20:48 urging immediate ER evaluation/911 guidance, and a follow-up message at 20:47 stated things appeared steady while remaining alert. No EPDS trend available (EPDS: none).

Overnight monitoring (2026-06-07 17:55 UTC to 2026-06-08 17:55 UTC) showed two stable BPs followed by a single critical hypertensive reading. BP at 15:40 and 16:00 were stable at 135/80 (pulse 72, both), remaining below ACOG Stage 1 threshold (>=140/90). At 17:19, BP acutely rose to 190/165 (pulse 70), exceeding ACOG Stage 2 criteria (>=160 and/or >=110). AI severity escalated from Moderate (score 0.15 at 15:41 and 16:01) to Severe (score 0.70 at 17:20) in parallel with the marked BP rise. Symptom screen remained low with 0 symptoms detected on all AI assessments, however the BP excursion represents a severe hypertensive event requiring prompt evaluation. One Severe Alert was generated at 17:20 and was acknowledged with SLA met; no additional resolution/documented follow-up is provided in the data. Care messages: at 16:00 and 16:01, SMS noted checks were stable and the care team would contact soon; at 17:20, an urgent SMS advised ER/911 immediately based on the alert.

Overnight monitoring (2026-06-07 17:50 UTC to 2026-06-08 17:50 UTC) demonstrated a critical BP deterioration after two stable readings. BPs at 15:40 and 16:00 were 135/80 mmHg (HR 72 each), consistent with no ACOG threshold crossing (below =140/90 Stage 1). At 17:19/17:20, BP acutely rose to 190/165 mmHg (HR 70), clearly exceeding ACOG Stage 2 criteria (=160/=110). AI severity increased from Moderate (score 0.15) at 15:41 and 16:01 to Severe at 17:20 (score 0.70), with the severe alert attributed to the marked Stage 2 range hypertension; symptom screen remained low (0 symptoms detected) in all assessments. A “Severe Alert” was issued at 17:20 and was acknowledged; escalation status shows SLA met, but clinical resolution of the hypertensive crisis is not documented in the provided data. Patient was sent SMS updates at 16:00 and 16:01 indicating stability, followed by urgent SMS at 17:20 advising immediate ED evaluation/911. No EPDS data were reported.

Overnight monitoring (2026-06-07 17:45 UTC to 2026-06-08 17:45 UTC) showed a critical hypertensive spike at 17:19 UTC with BP 190/165 (pulse 70). Earlier readings remained stable at 15:40 and 16:00 UTC: 135/80 (pulse 72) and 135/80 (pulse 72), which are below ACOG hypertension thresholds (no Stage 1/2 criteria met then). The 17:19 measurement clearly crosses ACOG Stage 2 (=160 systolic and/or =110 diastolic; also consistent with severe-range hypertension); this corresponded to AI severity escalation from Moderate (scores 0.15 at 15:41 and 16:01) to Severe (score 0.70 at 17:20). A “Severe Alert” was triggered at 17:20 and acknowledged, SLA met. Care messaging included two reassurance SMS at 16:00 and 16:01 indicating stability, followed by an urgent SMS at 17:20 advising immediate emergency evaluation/911. No symptoms were detected in the AI outputs, and no EPDS scores were recorded.

Overnight BP monitoring (2026-06-07 17:35 UTC to 2026-06-08 17:35 UTC) showed initial stability followed by a critical hypertensive event. Two readings remained stable: 135/80 at 15:40 and 135/80 at 16:00 (both pulse 72), below ACOG Stage 1 threshold (<140/90). At 17:19, BP acutely escalated to 190/165 (pulse 70), crossing ACOG Stage 2 criteria (=160/=110) and prompting severity escalation: AI verdicts changed from Moderate (score 0.15 at 15:41 and 16:01) to Severe (score 0.70 at 17:20) with “Severe Alert” generated at 17:20. Alert status is acknowledged and SLA met; no symptoms were detected in the AI assessment at any time point. SMS communications were sent at 16:00 and 16:01 indicating stability with expected follow-up, then an urgent message at 17:20 advising immediate ER evaluation/call 911. EPDS not reported/available.

Clinical context note: Only one severe BP value documented; trend was otherwise falling/stable until abrupt rise at 17:19. Confirm repeat BPs, assess for severe-range persistence and symptoms, and initiate guideline-based management for possible hypertensive emergency/superimposed preeclampsia.

Overnight monitoring is concerning for a hypertensive emergency-level value: BP 225/180 at 15:11 UTC with pulse 172. This exceeds ACOG Stage 2 thresholds (=160 systolic and/or =110 diastolic) and is consistent with the AI classification of Severity=Severe at 15:13 UTC (score 0.70). No alerts were recorded and no symptoms were detected on automated screen; however, severity is driven by the magnitude of BP rather than symptom burden. No escalation events, resolution documentation, or patient care messages were captured in the provided dataset. EPDS was not available/recorded for this period.

Overnight (2026-06-07 12:19 UTC to 2026-06-08 12:19 UTC), a single BP reading showed 145/100 at 11:44 UTC (HR 100). This meets ACOG Stage 2 criterion for diastolic hypertension (=100) despite systolic below 160. An AI severity classification labeled the episode as “Severe” (score 0.30) with note of “gestational: stage=second_trimester,” and no symptoms were detected. At 11:46 UTC, a “Severe Alert” was triggered and remains OPEN with SLA breached; no evidence of resolution or repeat confirming measurements is documented overnight. At 11:45 UTC, an urgent SMS was sent instructing the patient to seek emergency evaluation immediately or call 911. No EPDS scoring or trend information was provided during this interval.

Overnight monitoring (2026-06-07 11:59 UTC to 2026-06-08 11:59 UTC) showed one documented blood pressure of 145/100 mmHg at 11:44 UTC with pulse 100. This meets ACOG Stage 2 threshold for diastolic hypertension (=100) and crosses the =140/90 criterion for hypertensive range; however, no severe-range systolic BP (=160) was recorded. No symptoms were detected by the monitoring system at the time of measurement, and AI severity flagged “Severe” (score 0.30) with gestational categorization noted by the system as stage=second_trimester.

An alert was generated at 11:46 UTC (“Severe Alert — …”) with status Open and SLA pending. At 11:45 UTC, an urgent SMS was sent advising immediate evaluation in the nearest emergency department or to call 911 based on symptoms; resolution status of that outreach and patient clinical follow-through is not documented.

No EPDS data were available and no escalation resolution is recorded in the provided dataset.

Overnight monitoring (2026-06-07 11:54 UTC to 2026-06-08 11:54 UTC): Single BP documented at 11:44 UTC with 145/100 mmHg (pulse 100). This meets ACOG Stage 2 diastolic threshold (=100) but systolic remains below 160; no trend analysis possible given only one reading. AI severity flagged as “Severe” (score 0.30) despite low symptom burden (0 symptoms detected) and low overall risk stated by the model; gestational classification noted as “second_trimester.” An open “Severe Alert” was generated at 11:46 UTC (status: open; SLA pending). Patient received an urgent SMS at 11:45 UTC advising immediate ER evaluation/call 911 based on symptom-based triage; no EPDS assessments were recorded. No escalation resolution is documented overnight; alert remains open.

The tricky thing about blood pressure is while it can be too high, it can also be too low. There is precedent for this in the literature [1].

Overnight BP monitoring (19:56 UTC–05 Jun 19:56 UTC) is notable for a single critical hypertensive reading: 200/100 at 19:32 (pulse 100). This exceeds ACOG Stage 2 threshold (=160 systolic or =110 diastolic) and triggered a “Severe” AI alert at 19:32. Surrounding values were not persistently elevated: 148/96 at 18:15 (Stage 1 range =140/90), then 120/80 at 19:31 (normalized), followed by 121/79 at 19:36 (normalized). No symptoms were detected in the AI adjudications (score reflections driven by BP only); no EPDS reported. Two severe alerts were opened and remain open: 18:58 (SLA breached) and 19:33 (SLA pending), consistent with the hypertensive episode at 19:32. Two urgent SMS messages were sent at 18:27 and 19:33 instructing emergency evaluation; resolution status of escalation is not documented.

Overall pattern: brief spike to Stage 2 range with rapid return to baseline within minutes; however, the isolated severe-range BP warrants prompt clinical evaluation to exclude hypertensive emergency/secondary causes and to assess for recurrence/persistence.

Overnight monitoring (19:55 UTC–next day 19:55 UTC) showed a critical hypertensive spike at 19:32 UTC with BP 200/100 (pulse 100), which crosses ACOG Stage 2 threshold (=160 systolic and =110 diastolic). Surrounding BPs were lower: 18:15 UTC 148/96 (pulse 85) [meets ACOG Stage 1 only for diastolic (=90)], then 19:31 UTC 120/80 (pulse 72), followed by rapid normalization at 19:36 UTC 121/79 (pulse 72). No symptoms were detected on AI review throughout (AI symptom component 0), and gestational stage entered as second trimester. Two severe alert events were generated: 18:58 “Severe Alert” (status Open; SLA breached) and 19:33 “Severe Alert” (status Open; SLA pending), corresponding temporally to the concerning readings. Urgent care messages were sent to the patient at 18:27 UTC and again at 19:33 UTC advising emergency evaluation/911 if indicated. No EPDS data available. Overall trend: transient rise to Stage 2 followed by prompt return to normotension within ~4 minutes; however, the Stage 2 reading warrants immediate clinical assessment to exclude evolving hypertensive disorder or measurement artifact.

Overnight BP monitoring (19:50 UTC 6/4–19:50 UTC 6/5) showed a critical spike at 19:32: 200/100 (pulse 100), crossing ACOG Stage 2 threshold (=160/=110). Prior reading at 18:15 was 148/96 (pulse 85), meeting ACOG Stage 1 diastolic (=90) but not Stage 2. Subsequent BPs returned to normal range: 19:31 120/80 (pulse 72) and 19:36 121/79 (pulse 72), indicating an abrupt rise followed by rapid normalization.

No symptoms were detected by the monitoring logic across readings. AI severity escalated to “Severe” after the 19:32 BP (severity score 0.70) and was labeled severe at 18:15/18:46 with no BP escalation noted then. Two severe alert events were recorded (18:58 and 19:33) and remain open/pending, suggesting unresolved escalation workflow. Two urgent SMS messages were sent to the patient at 18:27 and 19:33 advising immediate ER evaluation or calling 911; resolution of patient response is not documented. No EPDS assessments were reported.

Recommendation: Same-day urgent clinician review is indicated given the transient ACOG Stage 2-range BP (200/100 at 19:32) despite subsequent normalization; assess for hypertensive disorders/secondary causes and ensure appropriate evaluation and management.

Overnight BP monitoring (2026-06-04 19:49 UTC to 2026-06-05 19:49 UTC) showed a critical spike to 200/100 at 19:32 (pulse 100), exceeding ACOG Stage 2 hypertension threshold (=160 systolic and/or =110 diastolic). Surrounding measurements were lower and near-normal: 148/96 at 18:15 (pulse 85; borderline Stage 2 diastolic not met for systolic but diastolic 96), 120/80 at 19:31 (pulse 72), and 121/79 at 19:36 (pulse 72). No symptoms were detected on AI review during readings, and no EPDS changes were available (EPDS not measured). Two severe alert events were triggered and remain open: 18:58 (SLA breached) and 19:33 (SLA pending). Escalation occurred via two urgent SMS messages at 18:27 and 19:33 advising emergency evaluation/911; resolution status is not documented in the provided data.

Overnight BP monitoring (19:45 UTC–19:45 UTC) showed one critical hypertensive reading followed by normalization. Most concerning: 200/100 at 19:32 UTC (pulse 100), which meets ACOG severe range (Stage 2 =160/=110). Preceding value at 18:15 was 148/96 (pulse 85) — Stage 1 threshold for diastolic (=90) but not Stage 2. Subsequent readings improved to 120/80 at 19:31 (pulse 72) and remained normal at 19:36 121/79 (pulse 72).

AI severity flagged highest risk at 19:32 (Severe, score 0.70) despite no symptoms detected; earlier/separate entries were marked Severe by the AI in association with chart/context alerts, but without a BP value exceeding thresholds other than the 19:32 episode. Alerts at 18:58 and 19:33 were both left Open (SLA breached/pending), corresponding to the severe alerts triggered during monitoring. No EPDS data available and no escalation resolution documented in the record provided. Two urgent SMS messages were sent (18:27 and 19:33) instructing immediate ER/911 evaluation.

Overnight monitoring (19:44 UTC 6/4–19:44 UTC 6/5) showed a critical hypertensive spike followed by normalization. BP readings: 148/96 at 18:15 (pulse 85) ? 120/80 at 19:31 (72) (transient improvement) ? 200/100 at 19:32 (100), crossing ACOG Stage 2 threshold (=160/=110) ? 121/79 at 19:36 (72). No symptoms were detected in associated AI assessments (0 symptoms throughout). AI severity escalated to “Severe” around the 200/100 reading (18:16 and 18:46 initially flagged as Severe despite limited symptom data; definitive Stage 2 risk at 19:32 with score 0.70), then downgraded to Low when subsequent BPs returned to near-normal (19:36).

Two alerts were triggered for “Severe Alert” and remain open/pending: 18:58 (status Open; SLA breached) and 19:33 (status Open; SLA pending). Two urgent SMS messages were sent at 18:27 and 19:33 advising emergency evaluation/911 in response to alerts.

No EPDS data available. No escalation resolution documented in the provided dataset.

Overnight BP monitoring (19:40 UTC 6/4 to 19:40 UTC 6/5) showed a critical hypertensive spike at 19:32: 200/100 (pulse 100). This exceeds ACOG Stage 2 thresholds (=160/=110) and is followed by normalization to 121/79 at 19:36 (pulse 72) with an intervening normal reading at 19:31: 120/80 (pulse 72). An additional elevated reading was recorded at 18:15: 148/96 (pulse 85), meeting ACOG Stage 1 for diastolic (=90) but not systolic (=140); otherwise pressures were not persistently elevated.

AI severity scoring flagged the 19:32 event as Severe (score 0.70) due to Stage 2 range BP, with no symptoms detected across readings. Earlier AI outputs also marked “Severe” alerts despite limited context, but only the 19:32 value represents clear ACOG threshold crossing.

Two severe alert events remain open: one at 18:58 (SLA breached) and another at 19:33 (SLA pending); current resolution status is unknown. Two urgent SMS messages were sent at 18:27 and 19:33 advising emergency evaluation/911 based on symptom screen (no symptoms documented by the monitoring AI).

No EPDS data available.

Overnight BP monitoring (19:39 UTC 6/4 to 19:39 UTC 6/5) shows one critical hypertensive reading: 200/100 at 19:32 (pulse 100), which crosses ACOG Stage 2 criteria (=160/110). Surrounding measurements were lower: 148/96 at 18:15 (Stage 1 range due to diastolic 96), then 120/80 at 19:31, and 121/79 at 19:36—overall pattern is transient severe spike with subsequent normalization (falling to Stage 0-range after the 19:32 event). No symptoms were detected by the AI triage at any timepoint (symptom score 0/low across assessments).
Two “Severe Alert” notifications were triggered (18:58 open/SLA breached; 19:33 open/SLA pending). Care team escalations via SMS occurred at 18:27 and 19:33 with urgent ER/911 instructions; resolution status of alerts is not documented. EPDS not recorded/changed.

Overnight BP monitoring (2026-06-04 19:35 UTC to 2026-06-05 19:35 UTC) shows an abrupt hypertensive spike with sustained concern for Stage 2 range. At 18:15 BP was 148/96 (pulse 85), which crosses ACOG Stage 1 threshold (=140/90) but not Stage 2. At 19:31 BP normalized to 120/80 (pulse 72) but then at 19:32 BP surged to 200/100 (pulse 100), clearly meeting ACOG Stage 2 (=160/=110). No symptoms were detected in automated assessments (0 symptoms across severity determinations).

AI risk stratification escalated to Severe at 19:32 (Score 0.70) citing Stage 2 BP; earlier 18:16 and 18:46 assessments were marked Severe despite limited data/SMS-based urgency context (no symptom signals). Severe alerts at 18:58 and 19:33 remain open (18:58 status Open, SLA Breached; 19:33 status Open, SLA Pending). Urgent SMS guidance was sent at 18:27 and 19:33 to seek emergency evaluation immediately/911.

No EPDS results available.

Overnight monitoring (06/04 19:34–06/05 19:34) shows a critical BP spike with subsequent improvement. Initial BP at 18:15 measured 148/96 (pulse 85), crossing ACOG Stage 1 threshold (=140/90). A later reading at 19:31 was 120/80 (pulse 72), returning to baseline/normal range, suggesting variability rather than persistent hypertension. However, a markedly severe value occurred at 19:32: 200/100 (pulse 100), exceeding ACOG Stage 2 threshold (=160/110) and corresponding to the highest-risk AI assessment (Severity=Severe, Score=0.70). No symptoms were detected in AI determinations (Symptom: Low; 0 symptoms) and EPDS was not assessed/available.

Two severe alerts were triggered: 18:58 (status Open; SLA breached) and 19:33 (status Open; SLA pending). Urgent escalation messages were sent to the patient at 18:27 and 19:33 instructing immediate ER evaluation/911 activation; resolution status of these alerts/communications is not documented as closed. No antihypertensive therapy or additional hypertensive history data were provided in the AI outputs.

Overnight (2026-06-04 19:30 UTC to 2026-06-05 19:30 UTC), single BP documented at 18:15: 148/96 mmHg (HR 85). This meets ACOG Stage 1 threshold by diastolic (=90) but does not reach Stage 2 (=160/110). No symptom reports were detected by the monitoring system. Two AI evaluations recorded “Severe” status with score 0.30: at 18:16 contextualized the available BP as high-risk but noted low symptom burden; at 18:46 severity remained severe despite no additional BP data and no antihypertensive medication information available in the record. A “Severe Alert” was triggered at 18:58 and remains open with SLA breached—resolution not documented. An urgent care message was sent at 18:27 advising immediate ER evaluation/911 based on symptoms; no EPDS data available to assess mood trajectory.

Overnight monitoring (19:24 6/4 to 19:24 6/5): Single BP recorded at 18:15 (UTC) = 148/96 with pulse 85. This meets ACOG Stage 1 hypertension threshold (=140/90) but does not reach Stage 2 (=160/110). No additional BP values were available overnight to assess trend (stable/ascending cannot be determined).

AI severity flagging occurred at 18:16 and 18:46 (both Severity=Severe, score 0.30). Despite “severe” system labeling, documented reasoning cited low risk and 0 symptoms detected; gestational context noted as second trimester. A separate system severity alert was generated at 18:58 (“Severe Alert”), status remains Open with SLA pending; no resolution documented.

Patient-facing escalation: an SMS message was sent at 18:27 instructing urgent ED evaluation/911 based on symptoms assessment, though the monitoring dataset reports 0 symptoms detected.

EPDS: not assessed/none recorded.

No other clinical escalation, medication changes, or resolution events were documented overnight.

Overnight (6/4 19:24 UTC–6/5 19:24 UTC), single BP measurement at 18:15 showed 148/96 mmHg (HR 85). This meets ACOG Stage 1 hypertension criteria (=140/90) but does not cross Stage 2 threshold (=160/110). No symptoms were detected by the AI on both assessments (severity flagged as “Severe,” but associated risk remained low). AI severity remained unchanged on repeat review at 18:46 (no additional BP values available). An alert was triggered at 18:58 (“Severe Alert”) and remains open with SLA pending; no documented resolution/closure overnight. A care message was sent at 18:27 via SMS with urgent ER/911 instructions based on the symptom screen (no EPDS data available; no EPDS trend to report).

Overnight monitoring (19:19 UTC to 19:19 UTC) shows a single documented blood pressure of 148/96 at 18:15 (pulse 85). This meets ACOG criteria for Stage 1 hypertension (=140/90) but does not reach Stage 2 (=160/110). No additional BP trend data are available overnight (no repeat readings recorded).

Clinical risk scoring by AI generated two “Severe” severity flags (18:16 and 18:46). In both assessments, symptom screen was negative (0 symptoms detected) and the documented BP was not clearly corroborated by other readings. One alert was opened at 18:58 (“Severe Alert—…”, status open; SLA pending).

Overnight monitoring (19:19 UTC 6/4 to 19:19 UTC 6/5) shows a single BP of 148/96 at 18:15 (pulse 85). This meets ACOG criterion for elevated BP/stage 1 range (=140/90) but does not reach stage 2 (=160/110). No hypertensive symptoms were detected (severity scoring driven by gestational context rather than BP trend); no additional BP readings were recorded overnight, so no rise/fall trend can be assessed. Two AI verdicts at 18:16 and 18:46 both labeled Severity=Severe with low-risk assessment and 0 symptoms detected, and gestational context marked as second trimester; however, objective data available are limited to the single reading above. One alert triggered at 18:58 (“Severe Alert”) and remains open with SLA pending; no documented resolution or escalation outcome. An urgent SMS was sent at 18:27 advising ER/911 based on symptoms, though concurrently 0 symptoms were detected by the system. No EPDS data available and no EPDS change can be reported.

Overnight monitoring (19:14 UTC to 19:14 UTC): BP recorded once at 18:15 UTC—148/96 mmHg, pulse 85. This meets ACOG criteria for Stage 1 hypertension (=140/90) but not Stage 2 (=160/110). No additional BP values were captured overnight to assess trend (stable vs rising/falling cannot be determined). No symptoms were detected by the AI screening, and no EPDS changes were recorded (no score available). Two AI assessments at 18:16 and 18:46 UTC both returned Severity=Severe (score 0.30) with “0 symptoms detected”; first assessment cited BP 148/96 and labeled “gestational: severe/stage=second_trimester,” while second assessment was based on missing additional BP data. A “Severe Alert” was triggered at 18:58 UTC and remains open (SLA pending), suggesting escalation is not yet resolved. An urgent SMS message was sent to the patient at 18:27 UTC advising immediate evaluation in the nearest emergency room or calling 911.

Recommendation: Same-day clinical review is indicated to confirm gestational age/stage labeling, repeat BP(s), and determine need for antihypertensive or preeclampsia evaluation given Stage 1 BP with an unresolved severe alert.

Overnight period (6/4 19:14–6/5 19:14): Notable BP 148/96 recorded at 18:15 UTC (HR 85). This meets ACOG threshold for elevated diastolic (stage 1 criteria: =140/90) due to diastolic 96; no stage 2 threshold (=160/110) observed. AI severity flags are marked “Severe” (score 0.30) at 18:16 and again at 18:46, but these were not accompanied by additional BP readings or documented symptoms (0 symptoms detected). At 18:58, an open “Severe Alert” remained unresolved (SLA pending). Care escalation: an urgent SMS was sent at 18:27 instructing evaluation at the nearest emergency room/call 911 based on reported symptom screen; no resolution documentation available. No EPDS data captured during this interval. Overall BP trend cannot be assessed beyond a single reading; values suggest stage 1-level hypertension risk requiring confirmation and monitoring.

Overnight 19:09 UTC (6/4) to 19:09 UTC (6/5), one BP recorded: 18:15 — 148/96 mmHg (HR 85). This meets ACOG hypertensive threshold for Stage 1 (=140/90) but does not reach Stage 2 (=160/110). No symptom data were detected, and no EPDS was recorded.

AI adjudication was repeatedly marked “Severe” (score 0.30) at 18:16 and again at 18:46; however, the only available objective finding was the single elevated BP above. At 18:58, an “Severe Alert” was triggered and remains Open with SLA pending. An urgent SMS message was sent at 18:27 advising emergency evaluation based on symptoms; resolution status is not documented, and no escalation outcome is recorded in the chart.

No trend assessment is limited by absence of additional BP measurements and lack of longitudinal vitals. Critical gap: determine whether additional hypertensive readings occurred and whether any red-flag symptoms (headache, visual changes, RUQ pain, dyspnea, edema) were present around the time of the alert.

Overnight monitoring (19:09 UTC 6/4–19:09 UTC 6/5) showed a single BP measurement of 148/96 at 18:15 UTC (pulse 85). This crosses the ACOG Stage 1 threshold (=140/90) but does not meet Stage 2 (=160/110). No symptom reports were detected by the AI (symptom score 0), and no EPDS data were provided.

AI severity classification flagged the episode as “Severe” at 18:16 UTC (Severity=Severe, Score=0.30; gestational stage noted as second trimester) and again at 18:46 UTC (Severity=Severe, Score=0.30). The rationale included low risk due to limited available BP context and no detected symptoms; however, the severity label appears discordant with the available BP magnitude (148/96) and should be interpreted cautiously given missing data in the second verdict.

An alert was generated at 18:58 UTC (“Severe Alert — …”), status remains Open with SLA pending. A care escalation message was sent to the patient at 18:27 UTC via SMS instructing urgent ED evaluation/911 based on symptoms (no symptoms detected in AI outputs; confirm whether patient reported symptoms or if this was templated). No escalation resolution is documented.

Overnight monitoring (2026-06-04 19:04 UTC to 2026-06-05 19:04 UTC) identified a single BP measurement of 148/96 at 18:15 UTC (HR 85). This meets ACOG criterion for Stage 1 hypertension (=140/90) but does not reach Stage 2 (=160/110). No symptom documentation was recorded by the AI (0 symptoms detected), and there were no EPDS assessments reported.

Two AI assessments returned “Severe” severity with low risk (score 0.30) at 18:16 and 18:46 UTC; however, the only available objective datum was the BP 148/96, and the second assessment occurred without an additional BP entry. An alert was generated at 18:58 UTC (“Severe Alert”), with status Open and SLA pending; no resolution event is documented.

A care message was sent to the patient at 18:27 UTC with urgent instructions to proceed to the nearest emergency room or call 911 “based on symptoms” (symptoms not substantiated in the provided dataset). No antihypertensive therapy or additional BP trend (rising/falling) can be determined beyond the single reading.

Overnight (2026-06-04 18:59 UTC to 2026-06-05 18:59 UTC), the most critical finding was a borderline severe-range BP reading: 148/96 at 18:15 UTC (HR 85). This meets ACOG Stage 1 threshold (=140/90) with no evidence of Stage 2 (=160/110) in the available data. No symptom documentation or EPDS changes were provided; AI symptom flags repeatedly reported 0 symptoms.

Two AI verdicts were generated (18:16 and 18:46) with Severity categorized as “Severe” but low clinical risk assigned (BP risk assessed as low given available data; no additional BP, hypertensive diagnoses, or antihypertensive medication details provided). An alert at 18:58 (“Severe Alert — …”) remains open with resolution pending (SLA pending).

Escalation/communication: an urgent SMS message was sent at 18:27 UTC advising emergency evaluation/911 based on symptom screening; symptom screen later reflected 0 symptoms, but the alert remains unresolved.

Recommendation: Same-day clinician review is warranted to reconcile the severity alert with the limited BP dataset and determine need for antihypertensive workup/management per hypertensive pregnancy protocols.

Overnight (2026-06-04 18:54 UTC to 2026-06-05 18:54 UTC), the critical data point was a single elevated BP: 148/96 at 18:15 (pulse 85). This represents ACOG Stage 1 hypertension threshold crossing by diastolic (=90/Stage 1), but does not meet Stage 2 (no =160 systolic or =110 diastolic) and there were no repeat readings to assess trend (single measurement; no rising/falling trend determinable).

Two AI severity outputs were recorded at 18:16 and 18:46 with severity labeled “Severe” and score 0.30; however, contextual reasoning indicates low risk with no detected symptoms and no hypertensive comorbidity/antihypertensive medication data available. No formal clinical alerts were triggered in the monitoring system.

Escalation: An urgent SMS message was sent at 18:27 advising emergency evaluation based on symptom screening (no symptoms were detected per AI summaries). No EPDS data available and no EPDS changes reported.

Care gaps/uncertainty: Lack of additional BP measurements and absence of symptom documentation limit interpretation of persistence or progression of hypertension overnight.

Overnight monitoring 2026-06-04 18:54 UTC to 2026-06-05 18:54 UTC: Single BP recorded at 18:15 — 148/96 mmHg (HR 85). This meets ACOG Stage 1 hypertension threshold (=140/90) with diastolic =90, but is below Stage 2 (=160/110). No additional BP values were captured overnight to assess trend (data limited to a single reading). No symptom detections were recorded by the AI screening (0 symptoms). AI severity flags were entered at 18:16 and 18:46 (Severity=Severe, Score=0.30), though risk was characterized as low; one entry attributed severity to gestational context while noting no symptoms and BP “within normal range,” creating an internal inconsistency given the documented 148/96. No escalation alerts were generated in the system (0 alerts). One urgent care message was sent to the patient at 18:27 via SMS advising immediate ER evaluation or calling 911 if symptomatic.

Overnight 18:49 UTC (6/4) to 18:49 UTC (6/5), only one BP documented: 18:15 UTC = 148/96 mmHg (HR 85). This meets ACOG Stage 1 hypertension threshold (=140/90) but not Stage 2 (=160/110). No additional BP readings were available overnight, so trend (rising/stable/falling) cannot be assessed. AI severity flags were recorded at 18:16 and 18:46 UTC as “Severe” (score 0.30), but both assessments documented no symptoms (0 symptoms detected) and low physiologic risk; the second entry was limited by absence of BP data and lacked information on antihypertensive use or hypertensive diagnosis confirmation. No alerts were triggered in the system (0 alerts). One urgent SMS message was sent at 18:27 UTC instructing the patient to present to the nearest emergency room or call 911 based on symptoms; however, no symptoms were captured in the AI verdicts and there is no EPDS data available.

Overnight monitoring (18:48 UTC 6/4 to 18:48 UTC 6/5) shows a single documented BP of 148/96 at 18:15 UTC (HR 85). This meets ACOG Stage 1 hypertension threshold (=140/90) but is below Stage 2 (=160/110). No additional BP trend data are available during the interval (no other readings recorded). No symptoms were detected on AI screening, and there were no recorded alerts requiring automated escalation. Despite the single Stage 1 BP, AI severity flagged as “Severe” (score 0.30) largely in the context of incomplete data/gestational stage; however, risk was assessed as low and no hypertensive diagnosis/antihypertensive therapy information was available in the provided dataset. An SMS message was sent at 18:27 UTC urging urgent ER evaluation based on symptoms (content indicates high urgency), but no symptom evidence is present in the data provided. EPDS was not reported overnight.

Overnight monitoring (18:44 UTC 6/4 to 18:44 UTC 6/5) showed a single elevated BP reading: 148/96 at 18:15 (HR 85). This meets **ACOG Stage 1 hypertension criteria** (=140/90) but does **not** reach Stage 2 thresholds (=160/110). No escalation alerts were triggered and there were no documented symptoms overnight. An AI severity assessment at 18:16 reported Severity=Severe (score 0.30), with risk deemed low and no symptoms detected; note that gestational context was flagged as second-trimester in the AI logic. A care message was sent at 18:27 via SMS with urgent ER/911 instructions based on symptom screening; however, symptom data in the AI readout did not indicate active symptoms. No EPDS change available (no scores reported).

Overnight BP: 148/96 at 18:15 UTC (HR 85). This meets Stage 2 threshold by diastolic (=110 is Stage 2; however diastolic here is 96, so overall BP does not cross Stage 1 threshold for either systolic =140 or diastolic =90? Systolic 148 is Stage 1; diastolic 96 is also Stage 1). No additional BP readings were recorded during the monitoring window, so trend assessment is limited (single value). No symptom data or escalation alerts were documented; AI severity flagged “Severe” (score 0.30) despite low symptom risk (0 symptoms) and no threshold crossing to =160/110. An urgent SMS was sent at 18:27 UTC instructing emergency evaluation/911 based on symptom-based triage (resolution status not documented). No EPDS assessments were recorded overnight. No alerts were generated in the system.

Overnight monitoring (18:39 UTC to 18:39 UTC): Single BP reading at 18:15 UTC showed 148/96 mmHg with pulse 85. This meets ACOG stage 1 threshold (=140/90) based on diastolic >90; not at stage 2 (=160/110). No additional BP trends available (only one value). No symptom data were detected, and no alerts were generated by the system. AI severity classification reported “Severe” (score 0.30) at 18:16 UTC, citing gestational context (stage indicated as second trimester) despite low risk/low symptom burden on the verdict. An urgent SMS was sent at 18:27 UTC instructing the patient to go to the nearest emergency room or call 911; escalation status/resolution is not documented. EPDS was not assessed/provided.

Overnight (2026-06-04 18:38 UTC to 2026-06-05 18:38 UTC), the only documented blood pressure was 148/96 mmHg at 18:15 UTC (HR 85). This meets ACOG Stage 1 range by diastolic (=90) but does not reach Stage 2 (=160/110). No trend assessment is possible due to single reading; no escalation alerts were triggered. At 18:16 UTC, an AI verdict flagged severity as “Severe” (score 0.30) but noted low symptom burden (0 symptoms detected) with gestational context labeled “second_trimester” and BP considered within the reported “normal range” by the model. No additional vitals, symptoms, or laboratory data are available in this interval. No EPDS assessments were recorded. An urgent SMS was sent at 18:27 UTC advising evaluation at the nearest ER or calling 911 based on “symptoms”; however, no symptoms are documented in the provided dataset and no alert reconciliation is available. No resolution of escalation is documented overnight.

Overnight monitoring from 2026-06-04 18:33 UTC to 2026-06-05 18:33 UTC: one BP reading recorded at 18:15 with 148/96 (HR 85). This is above the ACOG Stage 1 threshold for diastolic (=90) but below Stage 2 (no =160/110). No additional BP trend data available overnight (single measurement; cannot assess rising/falling). AI alerting classified overall severity as “Severe” (score 0.30) despite low detected symptom burden (0 symptoms) and no active alerts logged; chart reasoning notes gestational context as second trimester. No EPDS changes documented. One care escalation message was sent at 18:27 via SMS instructing urgent ED evaluation/call 911 based on symptom triage, but symptom data were not present in the provided dataset and no escalation resolution status is recorded.

Key gaps for morning review: confirm gestational age, obtain symptom review (headache, visual changes, RUQ pain, dyspnea), and correlate with any subsequent/confirmatory BPs or labs not included here.

Overnight BP monitoring (2026-06-04 17:27 UTC to 2026-06-05 17:27 UTC) showed a critical transient severe outlier followed by partial improvement. Most recent readings: 17:22 BP 148/96 (pulse 85) — diastolic remains elevated (ACOG Stage 1 threshold reached for diastolic =90) but below Stage 2 (no systolic =160 or diastolic =110 documented). Prior readings: 17:16 BP 135/85 (pulse 80) consistent with Stage 1 by diastolic only; 17:15 BP 120/80 (pulse 100) normal.

Most concerning event occurred at 17:16 BP 195/180 (pulse 150), which exceeds ACOG Stage 2 thresholds (=160/=110) and was classified as Severe by the monitoring system. No symptoms were detected in the associated AI assessments. An alert was generated at 17:18 (“Severe Alert”) and remains Open with SLA pending; escalation resolution is not documented. Two urgent SMS messages were sent at 17:18 advising emergency evaluation/calling 911 and then a follow-up message indicating “moderate for now.”

EPDS: not obtained/reported.

Overnight monitoring (2026-06-04 17:23 UTC to 2026-06-05 17:23 UTC) was notable for a critical hypertensive reading at 17:16 (195/180, pulse 150), which exceeds ACOG Stage 2 threshold (=160/=110) and triggered a Severe AI severity (score 0.70). Subsequent measurements did not confirm persistent severe hypertension: BP at 17:22 was 148/96 (pulse 85), remaining in severe classification per the AI but meeting only Stage 1 diastolic criteria (=140/90 crossed via diastolic 96). Earlier BPs were normal-to-mildly elevated: 17:15 120/80 (pulse 100) and 17:16 135/85 (pulse 80). No symptoms were detected on AI review (0 symptoms across readings). An alert (“Severe Alert …” at 17:18) remains Open with SLA pending; care escalated with two urgent SMS communications at 17:18 advising emergency evaluation/911 and notifying that findings were “moderate for now.” No EPDS assessments were recorded.

Overnight BP monitoring (2026-06-04 17:22 UTC to 2026-06-05 17:22 UTC) shows a critical hypertensive outlier followed by persistent elevation. Readings: 17:15 120/80 (HR 100) and 17:16 135/85 (HR 80) were below ACOG Stage 1 threshold. At 17:16, a marked spike occurred: 195/180 (HR 150), which meets/exceeds ACOG Stage 2 criteria (=160/=110). Subsequent BP at 17:22 was still elevated at 148/96 (HR 85), crossing ACOG Stage 1 threshold by diastolic (=90) but not meeting Stage 2. AI severity classification escalated from Low (17:15, score 0.00) to Moderate (17:16, score 0.15) and then to Severe (17:17, score 0.70) corresponding to the 195/180 event. No symptoms were detected in associated AI assessments. A “Severe Alert” was generated at 17:18 with status Open and SLA Pending. Two SMS messages were sent at 17:18: one requesting urgent ER evaluation/911 if symptomatic (triage language), and a follow-up message stating it “looks moderate for now” after review; resolution of the open alert is not documented. No EPDS data available.

Overnight monitoring (2026-06-04 17:18 UTC to 2026-06-05 17:18 UTC) is dominated by a critical hypertensive spike. At 17:16 UTC, BP recorded 195/180 with pulse 150; this meets/exceeds ACOG Stage 2 severe-range threshold (=160 systolic and/or =110 diastolic) and was the only reading crossing severe criteria. Preceding values showed improvement/stability: 120/80 at 17:15 UTC and 135/85 at 17:16 UTC (both below ACOG thresholds). No symptoms were detected in AI adjudications for any interval; severity rating increased from Low (score 0.00 at 17:15) ? Moderate (score 0.15 at 17:16) ? Severe (score 0.70 at 17:17) corresponding to the severe BP event.

An alert was generated at 17:18 UTC (“Severe Alert”; status open; SLA pending). Two SMS messages were sent at 17:18 UTC: one with urgent instruction to go to the nearest emergency room/call 911, and a follow-up message indicating it “looks moderate for now.” No EPDS assessments were documented.

Overnight review (2026-06-04 13:12 UTC to 2026-06-05 13:12 UTC): Elevated BPs noted early with subsequent normalization. At 12:18 BP 158/80 (HR 72) and at 12:20 BP 152/80 (HR 72). These readings approach ACOG Stage 1 threshold (=140/90), but diastolic remained <90 and systolic never reached =160/110; no ACOG Stage 2 crossing documented. By 12:35 BP returned to 120/80 (HR 72), representing a clear downward trend back to baseline.

System severity outputs were inconsistent with the recorded BP values: AI flagged “Severe” at 12:19 and 12:20 (score 0.60) citing preeclampsia flags despite “risk low” with BP values in the normal range; an alert was generated at 12:19 (“Severe Alert” status open, SLA breached). At 12:33 AI remained “Severe” (score 0.60) but without symptom flags; at 12:35 AI downgraded to “Low” (score 0.00).

Escalation: 12:19 severe alert opened; resolution status not documented (remains open).

Patient communication: 12:19 SMS sent instructing urgent ED evaluation/call 911 based on symptoms.

Instructions

by Sebastian Zhang | on Camila Maria Lopez | 3 years ago | 0 comments

Fluconazole (Diflucan), Itraconazole (Sporonox), and Ketoconazole

These medications are antifungal medications used to treat ringworm, athlete’s foot, yeast infections, tinea versicolor, nail fungus, and other fungal infections. They are all dosed a little differently so please pay attention to the instructions on the bottle so that the medication is taken properly. Taking them with an acidic beverage such as coke or orange juice will improve absorption into the blood stream.

Side effects include:
  1. Increased liver enzymes and liver damage
    1. This is more common with ketoconazole and itraconazole.
    2. Long term therapy (> 1 month) will require blood tests to evaluate liver enzymes.
  2. Drug interactions
    1. Your doctor will review your active medication list to ensure there are no interactions with these medications. Please let all of your physicians know when you are taking one of these medications.
  3. Nausea, vomiting, diarrhea, upset stomach
  4. Headaches
  5. Rashes
  6. Abnormal taste sensation
    1. This is a side effect of fluconazole.
  7. Worsening of heart failure and fluid retention
    1. This is a side effect of itraconazole.
