Overnight BP surveillance (2026-06-07 18:51 UTC to 2026-06-08 18:51 UTC) shows an abrupt hypertensive spike followed by persistent severe-range readings. Initial BPs at 15:40 and 16:00 were stable at 135/80 mmHg (pulse 72) with no symptoms flagged (AI severity Moderate, score 0.15; Stage: second trimester).
 
At 17:19–17:20 there was a critical deterioration: 190/165 mmHg (pulse 70) meeting ACOG Stage 2 criteria (>=160/110). A severe alert was issued at 17:20 (acknowledged; SLA met) and urgent ED/911 instructions were sent to the patient.
 
Subsequently, BP remained in the severe range: 155/140 mmHg at 18:07 (pulse 100), again exceeding ACOG Stage 2 threshold by diastolic (>=110); severe alerts were reissued at 18:08 and 18:14 (both acknowledged; SLA met). By 18:14, symptom burden increased to “severe” with 6 symptoms flagged on a preeclampsia screen (preeclampsia flag), corresponding to the highest AI severity (Severe, score 1.00). Care messages escalated accordingly with repeated urgent ED/911 instructions at 17:20, 18:08, and 18:14. No EPDS data reported.
